Abstract

A small controlled trial suggests that adding oral montelukast to inhaled beta-agonist therapy may be beneficial in acute asthma in children.

The authors note that urine levels of cysteinyl leukotrienes are raised in children with asthma, especially during exacerbations, and that leukotriene antagonists have shown benefits in chronic asthma. Based on the increased leukotriene levels seen in exacerbations, they speculate that such drugs might therefore have benefits in this situation. The trial involved 51 children aged between 2 to 5 years who were having a mild to moderate exacerbation of asthma. They were randomised to treatment with one tablet of montelukast 4mg or placebo in addition to standard therapy with inhaled beta agonist, and were followed up for four hours; outcomes were pulmonary index score, respiratory rate and pulse at baseline and for the duration of follow-up.

The results showed that patients treated with montelukast had consistently lower pulmonary index score and respiratory rate than those treated with placebo from 90 minutes after administration up to the end of the study. They were also less likely to require oral corticosteroids, although the difference was not statistically significant. The authors conclude that oral montelukast has potential as a useful adjunct to treatment with inhaled beta agonist therapy in children of this age group with mild to moderate exacerbations of asthma.

[Editor's note: this small study provides a useful pointer to a possible effect that needs to be confirmed in a larger trial. Montelukast is not licensed for use in acute asthma.]
